Role Overview
Seeking a Physical Therapist (PT) to provide home health care services, enabling patients to recover in their own homes. The role involves conducting initial and ongoing assessments of mobility, strength, and function, and developing personalized care plans with clear goals and outcomes. The PT delivers prescribed therapies, documents progress accurately, and adjusts treatment plans as needed. Responsibilities include discharge planning, patient and caregiver education, coordinating with healthcare teams, supervising staff (PTA, HHA), and completing assessments like OASIS where permitted. The position requires a degree in Physical Therapy from an accredited program, a current unrestricted license, and valid CPR. Preferred candidates have home health experience and OASIS documentation skills.
