Home Health nurse practitioner

Join our healthcare team as a Home Health Nurse Practitioner providing compassionate, personalized care in patients’ homes.

Conduct patient assessments, develop care plans, and perform clinical procedures such as IV infusions, wound suturing, and catheterization.

Educate patients on disease management, medication, and lifestyle modifications.

Utilize EHR systems like Epic or eClinicalWorks for documentation and coordinate with multidisciplinary teams for seamless care.

Administer treatments, perform diagnostic procedures, and support patient independence.

Requirements: Current NP license, experience in critical care preferred, proficiency with EMR/EHR platforms, and ability to work independently.
