Join our healthcare team as a Home Health Nurse Practitioner providing compassionate, personalized care in patients’ homes.
Conduct assessments, develop care plans, and perform treatments such as IV infusions, wound suturing, and medication management. Perform clinical procedures including catheterization, phlebotomy, and basic life support. Educate patients on disease management, medications, nutrition, and lifestyle changes. Utilize EHR systems for accurate documentation and coordinate with multidisciplinary teams for seamless care. Work independently with collaboration within healthcare teams. Requirements: Valid Nurse Practitioner license, experience in critical care preferred, proficiency with EMR/EHR and telehealth, and strong assessment and communication skills.
