Licensed Practical Nurse (LPN)
Provide compassionate, person-centered nursing care in home and community-based settings. Deliver direct care, administer medications, monitor vital signs, and support individuals with diverse medical and behavioral needs.
Key Responsibilities
- Implement individualized care plans in collaboration with RNs, therapists, and caregivers
- Administer medications and perform basic clinical monitoring
- Observe, report, and document health status changes
- Maintain accurate clinical documentation in an EHR system
- Support individuals’ safety and wellbeing while following clinical standards and compliance requirements
Qualifications & Skills
- Active LPN license
- Strong clinical skills in medication administration and vital sign monitoring
- Ability to work effectively with a multidisciplinary care team
- Comfort providing care to individuals with complex medical and behavioral needs
- Reliable documentation skills and knowledge of clinical compliance expectations
