Role: Licensed Practical Nurse (LPN)
Responsibilities:
- Provide direct in-home care to elderly, chronically ill, and disabled clients
- Perform assessments and monitor vital signs
- Administer medications and support wound care
- Implement care plans under RN and physician supervision
- Educate patients and families about health management
- Document care activities accurately in Electronic Health Records (EHR)
