We are seeking a Speech Language Pathologist to join our Home Care team, providing personalized evaluations and treatments in patients’ homes to improve communication, cognition, and swallowing skills.
Responsibilities include interpreting medical prescriptions, conducting assessments, developing and adjusting evidence-based treatment plans, documenting progress, educating patients and caregivers, and collaborating with an interdisciplinary team.
Qualifications include a Master’s degree in Speech-Language Pathology, CCC or CFY, NY state licensure, and CPR certification; experience with dysphagia and neurogenic disorders is preferred.
