Seeking a Speech Language Pathologist for our Home Care team to provide individualized assessments and treatments in patients’ homes, focusing on communication, cognition, and swallowing.
Responsibilities include interpreting medical prescriptions, conducting assessments, developing and adjusting personalized treatment plans, documenting progress, and educating patients and caregivers. Collaboration with interdisciplinary teams and participation in care planning are essential.
Minimum qualifications: Master’s degree in Speech-Language Pathology, valid NY State license, CCC from ASHA or CFY candidate, and CPR certification. Preferred experience with dysphagia and neurogenic disorders.
