Seeking a Speech Language Pathologist to provide personalized evaluation and treatment in patients’ homes, focusing on communication, cognition, and swallowing to promote safety and independence.
Conduct assessments, develop evidence-based treatment plans, and adjust strategies based on patient progress.
Document all activities in compliance with regulations and provide caregiver education on communication and swallowing techniques.
Collaborate with interdisciplinary teams and participate in team meetings for coordinated care.
Maintain licensure, stay current with clinical practices, and mentor peers as needed.
Qualifications include a Master’s degree, CCC from ASHA or CFY, NY State licensure, and CPR certification. Preferred experience includes working with dysphagia and neurogenic disorders.
