Home Care Speech Language Pathologist, Chenango County Region

Seeking a Speech Language Pathologist to join our Home Care team, providing personalized evaluations and treatments in patients’ homes to improve communication, cognition, and swallowing for safety and independence.

Conduct assessments and develop evidence-based, individualized care plans tailored to home environments.

Adjust treatment strategies based on patient progress and caregiver input.

Document all interventions and progress, ensuring compliance with regulations and reimbursement needs.

Educate patients and caregivers on communication techniques and safe swallowing practices.

Collaborate with interdisciplinary teams and participate in care planning and team meetings.

Maintain licensure, stay current with clinical best practices, and contribute to team development.

Qualifications include a Master’s in Speech-Language Pathology, CCC or CFY, NY licensure, and CPR certification. Preferred experience includes working with dysphagia and neurogenic disorders.
