Role: Provide patient-centered nursing care in a primary care setting.
Key duties:
- Assess patients and perform triage to determine appropriate care needs.
- Manage chronic conditions and deliver skilled nursing procedures.
- Conduct medication reviews and support safe medication use.
- Coordinate and support diagnostic testing and follow-up as ordered.
- Provide patient education to improve health understanding and self-management.
- Collaborate with providers and the care team to coordinate complex care.
- Support safe transitions from hospital to clinic and enhance continuity of care.
- Promote preventive care and contribute to improved patient outcomes.
Required qualifications/skills: Strong clinical assessment and triage skills, expertise in chronic care management, medication review, patient education, and effective teamwork/communication to coordinate complex care.
