Responsibilities include
providing high-quality clinical care and patient follow-up, developing clinical programs, collaborating with specialists and care teams, contributing to quality improvement initiatives, and
offering health education and counseling.Qualifications include
CA Medical License and Board Certification Eligibility, experience with diverse populations, strong clinical skills, preferably Spanish speaking, and
familiarity with electronic health records and community resources.
