Family Medicine Physician – Live Well Homecare

We are seeking a Family Medicine Physician to provide comprehensive, holistic primary care for patients of all ages, including diagnosing, treating illnesses, performing preventive services, and coordinating specialist care.

Perform medical assessments, physical exams, and minor procedures.

Manage chronic conditions such as diabetes and hypertension.

Educate patients on disease prevention and healthy lifestyles.

Coordinate care with specialists and maintain detailed records.

Provide pediatric and, in some settings, prenatal/postnatal care.

Qualifications include an Illinois medical license or eligibility, Board Certification or Eligibility in Family Medicine, and an MD or DO degree. Strong communication skills, cultural sensitivity, and organizational abilities are essential.

The role offers a full-time schedule (Monday–Friday, 8 am–5 pm) with clinical and scholarly responsibilities, a comprehensive benefits package, and a salary range of $225,000–$295,000. In-person work location.
