Family Medicine Physician – Live Well Homecare

The Family Medicine Physician provides comprehensive, holistic care for patients of all ages, including diagnosis, treatment, preventive services, and minor procedures.

Perform physical exams, manage chronic conditions, and coordinate specialist care.

Educate patients on health and lifestyle management.

Provide pediatric and prenatal/postnatal care as applicable.

Must hold an Illinois medical license, be board-certified or eligible in Family Medicine, and have MD or DO degrees.

Work Monday to Friday, with 3-4 clinical half-days and scholarly time.
