Family Medicine Physician – Live Well Homecare

The Family Medicine Physician provides comprehensive primary care across all ages, diagnosing and treating illnesses, managing chronic conditions, and performing minor procedures.

Perform assessments, physical exams, preventive care, and in-office procedures.

Educate patients on health and wellness, coordinate care with specialists, and maintain detailed medical records.

Provide pediatric and, when applicable, obstetric care.

Require Illinois medical license or eligibility, Board Certification or eligibility, and MD/DO degree.

Work Monday–Friday, with 3-4 clinical half-days weekly and time allocated for scholarly activities.
