The Advanced Practice Provider (APP) provides comprehensive outpatient family medicine care under supervision at a clinic in Prineville, OR.
Practices evidence-based diagnosis, treatment, and preventive care for patients of all ages within a Medical Home model. Coordinates multidisciplinary services, creates integrated treatment plans, and monitors patient progress, referring to specialists as needed. Prescribes medications, interprets diagnostics, and documents care efficiently in the EHR. Participates in call rotations, collaborates with healthcare teams, and supports quality improvement initiatives. Maintains licensure, certifications, and compliance with policies; engages in ongoing education. Requires a Master’s in Nursing or Physician Assistant Studies, relevant licensure, experience in outpatient family medicine, and team-based care skills.
