Advanced Practice Provider role providing outpatient primary care in a Family Medicine Medical Home setting at Prineville Clinic.
Responsibilities include evaluation, diagnosis, treatment, and documentation for patients of all ages, coordinating multidisciplinary services, creating comprehensive treatment plans, and monitoring ongoing care.
Work involves a minimum of 40 hours/week, with scheduled patient contact and participation in a shared call rotation, including evenings, weekends, and holidays.
Supported by a team including behavioral health, pharmacy, and care management to deliver evidence-based, personalized care.
Qualifications include a Master’s degree in Physician Assistant Studies or Nursing, current licensure, relevant certifications, and outpatient family medicine experience. Preferred candidates have 2+ years of practice, knowledge of Medical Home model, and experience with EHR systems like Epic.
Compensation ranges from $154,568 to $165,351 annually, with benefits such as health insurance, retirement plans, and paid time off. Relocation to Prineville, OR, is required.
