This full-time Advanced Practice Provider (APP) provides comprehensive outpatient primary care at a Family Care Clinic in Prineville, OR, under physician supervision.
Performs evaluation, diagnosis, treatment, and documentation for patients of all ages within a Medical Home model. Coordinates multidisciplinary services including behavioral health, pharmacy, nursing, and specialists to optimize patient outcomes. Creates and monitors individualized treatment plans, incorporating behavioral health, therapy, nutrition, and surgical interventions as needed. Participates in clinic call rotation, supporting after-hours patient care with backup from physicians. Practices evidence-based family medicine, collaborates with care teams, and ensures timely documentation and diagnostic reporting. Provides patient education, supports quality improvement initiatives, and maintains compliance with licensing, certifications, and organizational policies. Requires a master’s degree in Nursing or Physician Assistant Studies, current licensure, relevant experience in outpatient family medicine, and certifications in BLS/ACLS as applicable.
