This role is for an Advanced Practice Provider (APP) at a Family Care Clinic in Prineville, OR, supervised by a physician.
They will deliver outpatient medical care across all ages, including evaluation, diagnosis, treatment, and documentation, within a Medical Home model that integrates Behavioral Health, pharmacy, care management, and other services.
The APP will work at least 40 hours/week with a minimum of 36 hours of patient contact, participate in a shared call rotation, and collaborate with a multidisciplinary team to ensure comprehensive, patient-centered care.
Responsibilities include practicing evidence-based family medicine, prescribing medications, interpreting tests, providing patient education, and supporting quality improvement initiatives.
Qualifications include a Master’s degree in Physician Assistant Studies or Nursing, valid licensure, certifications (e.g., DEA, BLS, ACLS if applicable), and outpatient care experience. Preferred candidates have 2+ years in family medicine, familiarity with the Medical Home model, and Epic EHR experience.
