Seeking an Advanced Practice Provider to deliver outpatient primary care at a Family Medicine clinic in Prineville, OR.
Provides comprehensive evaluation, diagnosis, treatment, and documentation for patients of all ages, coordinating services within a Medical Home model. Works under supervision, collaborating with a multidisciplinary team to create personalized treatment plans incorporating behavioral health, pharmacy, therapy, and other services. Participates in clinic call rotation, after-hours duties, and maintains timely documentation and diagnostic reporting. Ensures evidence-based, patient-centered care, adheres to licensing, certification, and regulatory standards, and fosters a healing environment. Requires a Master’s in Nursing or Physician Assistant Studies, valid licensure, certification, and relevant outpatient experience. Preferred candidates have 2+ years in family medicine and familiarity with Medical Home and EHR systems. Compensation ranges from $154,568 to $165,351 annually, includes benefits such as health, dental, vision insurance, 401(k), and paid time off. Relocation to Prineville is required.
