The licensed CORE provider will deliver face-to-face mobile services to individuals with mental health and substance use disorders in a community setting.
Provide time-limited, goal-oriented support using person-centered and solution-focused interventions.
Identify treatment strategies to reduce symptoms and environmental stressors.
Offer supportive counseling, emotional/behavioral management, and problem behavior analysis to individuals and families.
Utilize strengths-based planning and help clients recognize triggers to maintain community stability.
Implement evidence-based psychotherapeutic interventions and collaborate with support networks.
Maintain accurate documentation per standards and participate in supervision and professional development.
Qualifications include licensure (LMSW, LCSW, LMHC, etc.), minimum three years’ community experience, strong communication skills, cultural responsiveness, and proficiency with EHRs.
Preferred candidates include bilingual individuals and those with personal recovery experience. Valid NY driver’s license required.
