The licensed CORE provider will deliver direct, face-to-face mobile services to individuals with mental health and substance use disorders in a community setting.
They will offer goal-directed, person-centered support, including counseling, solution-focused interventions, and behavioral management to help clients achieve recovery and wellness.
Key responsibilities include identifying treatment strategies, facilitating strengths-based planning, supporting emotional regulation, and implementing evidence-based practices. They will also assist families and support networks, maintain accurate documentation, and participate in supervision and training.
Qualifications require a license (e.g., LMSW, LCSW, LMHC, RN) with at least three years of community-based experience, strong interpersonal skills, cultural responsiveness, and proficiency with EHRs. Bilingual candidates and those with personal recovery experience are encouraged.
