Complex Care Navigator

We are seeking a Complex Care Navigator to support patients with serious illnesses by guiding them through the healthcare system and aligning care with their goals and values.


Key responsibilities include:

  • Enrolling patients into palliative care programs and serving as a primary contact for patients and families.
  • Providing education on program services and conducting community outreach, including home visits.
  • Building trusting relationships and facilitating communication among patients, families, and healthcare teams.
  • Supporting care coordination and ensuring patient-centered care within an interdisciplinary team.
  • Managing confidential patient information in compliance with HIPAA and organizational policies.
  • Maintaining organizational skills, emotional intelligence, and the ability to work independently and collaboratively.

  • Qualifications include:

    • High School Diploma or GED.
    • At least 2 years of customer service experience and proficiency with Microsoft Office.
    • Reliable transportation and a valid driver’s license with insurance.
    • Preferred healthcare-related training or experience, especially with complex or serious illness populations.

    Skills required:

    • Strong organizational, time management, and multi-tasking abilities.
    • Excellent communication skills, both verbal and written.
    • Ability to work under pressure and handle emotionally complex situations.
    • Demonstrated teamwork and good judgment.
