Role: Field Based Community Health Worker
Responsibilities: Assess and develop individualized care plans, coordinate with healthcare providers and community resources, facilitate referrals for social services, advocate for patient needs, document assessments and progress, engage members via in-person or telephonic methods, and support post-discharge activities. Qualifications & Skills: High school diploma or GED, 2+ years of field experience, bilingual in Spanish and English, reliable transportation, computer proficiency, and knowledge of community resources. Preferred qualifications include behavioral health experience, care management skills, and understanding of Medicaid populations. Soft skills include strong organization, communication, multitasking, and ability to work independently in a fast-paced environment.
