Community Health Worker supporting Medicare patients in underserved communities.
In this role, you will:
- Conduct patient outreach and build trusting relationships.
- Connect patients to primary care and essential social services.
- Provide health education and support adherence to care plans.
- Help reduce barriers to care, including transportation, housing, and food insecurity.
- Work with an interdisciplinary care team to monitor patient progress.
- Document activities and patient information in the EHR.
- Promote preventive care and help improve patient outcomes.
Key qualifications/skills: strong communication and relationship-building, ability to assess needs and coordinate resources, empathy and patient-centered mindset, teamwork, and proficiency with electronic health record documentation.
