Seeking a Registered Nurse (RN) for a community health setting, providing compassionate, patient-centered care across the lifespan, including primary and behavioral health support.
Key duties
- Conduct patient assessments and perform triage as needed
- Administer vaccinations and support preventive care
- Coordinate care and assist with referrals and follow-up
- Manage chronic conditions and support care planning
- Provide patient education and self-management support
- Document accurately in the EHR
- Participate in quality and safety initiatives
- Collaborate with an integrated care team, including providers, behavioral health, and dental staff
Qualifications/skills
- Current RN licensure
- Strong assessment, triage, and patient education skills
- Experience or comfort with primary and behavioral health nursing
- Proficiency with EHR documentation and care coordination
- Commitment to equitable care for underserved populations
