Role: Clinical Assessment RN
Responsibilities:
- Perform comprehensive in-home assessments using UAS tools for diverse patients.
- Evaluate medical, functional, and psychosocial needs to develop person-centered care plans.
- Collaborate with interdisciplinary teams to optimize patient services.
- Support safe transitions and ensure regulatory compliance.
- Provide education to patients and caregivers to promote understanding and engagement.
Qualifications and Skills:
- Registered Nurse (RN) licensure.
- Experience in clinical assessment and in-home care settings.
- Strong interpersonal and communication skills.
- Ability to collaborate effectively with diverse healthcare teams.
- Knowledge of regulatory requirements and care planning processes.
