The Certified Family Nurse Practitioner provides compassionate primary care services under physician supervision, focusing on wellness exams, acute illness, and disease management.
Position Summary: The NP delivers patient-centered care, assessing, diagnosing, and managing both acute and chronic conditions while emphasizing prevention and continuity of care. The role aims to improve health outcomes for underserved populations facing social barriers.
- Provide comprehensive primary care across the lifespan, including exams, preventive care, and chronic disease management.
- Collaborate with healthcare team members to ensure holistic, coordinated care.
- Maintain accurate documentation in the EHR and participate in quality improvement initiatives.
- Support health promotion and disease prevention efforts in underserved communities.

Salary: $90,000 – $100,000/year
