Care Manager

Role Overview

The Lead Caregiver is a frontline leader responsible for providing exceptional resident care while coaching and supporting a caregiving team.
They lead shift assignments, manage attendance, and coordinate coverage to ensure smooth operations.
Hands-on support with Activities of Daily Living (ADL), reviewing charting, and ensuring alignment with Individualized Service Plans (ISP) are key duties.
Monitoring resident changes and communicating concerns promptly are essential.
They also support onboarding and mentor new team members, promoting safety, compliance, and professionalism.

Qualifications & Skills

  • High School Diploma/GED; age 18+
  • CPR, First Aid, and Medication Administration certifications
  • Strong communication, delegation, and decision-making skills
  • Experience in assisted living, memory care, or senior care environments

Key Responsibilities

  • Lead shift operations and team coverage
  • Provide hands-on resident care as needed
  • Coach team members on quality, documentation, and hospitality standards
  • Review and ensure accurate charting aligned with ISPs
  • Monitor resident health and communicate concerns
  • Support team onboarding and mentoring
  • Promote safety, compliance, and effective shift transitions
