APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA providing independent, comprehensive medical care in long-term care facilities.

They aim to improve patient outcomes, enhance the healthcare experience, prevent unnecessary hospitalizations, and reduce costs.

Responsibilities include conducting exams, diagnosing and treating illnesses, developing care plans, providing preventive services, and managing chronic conditions.

They work autonomously within scope of practice, collaborating with facility leadership and interdisciplinary teams, while ensuring compliance with regulations and maintaining accurate documentation.

Additional duties involve infection control, health education, and safety oversight. The role requires strong clinical decision-making, communication skills, proficiency with EHR and telemedicine tools, and physical ability to respond quickly in emergencies.

Qualifications include a relevant degree, 2+ years of experience (long-term care preferred), and appropriate licensure/certification.
