APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time APRN or PA providing independent, collaborative medical care in long-term care facilities.

  • Manage patients’ comprehensive health needs through exams, diagnoses, treatments, and preventive services, focusing on improving outcomes and reducing hospitalizations.
  • Work autonomously within scope, making clinical decisions and escalating as needed, while building strong relationships with facility leadership and interdisciplinary teams.
  • Ensure compliance with regulations, maintain accurate documentation, and participate in quality initiatives.
  • Educate staff and patients on health promotion, ensure safety protocols, and utilize EHR and telemedicine tools.

    Qualifications include a relevant degree, at least 2 years of experience (preferably in long-term care), and active APRN or PA licensure. Benefits include health coverage, retirement plans, paid time off, and more. The role requires in-person work across statewide facilities.
