APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant who independently manages patient care in PALTC facilities while collaborating with staff and leadership.

Responsibilities include conducting examinations, diagnosing, treating illnesses, prescribing medications, developing care plans, and providing preventive services. The AP works autonomously within scope, making clinical decisions and serving as the primary provider, escalating as needed.

They build strong interdisciplinary relationships, ensure regulatory compliance, maintain accurate documentation, and participate in quality improvement. The role emphasizes health and safety oversight, staff and patient education, and working independently with excellent communication skills.

Proficiency in EHR and telemedicine tools, along with relevant licensure, certification, and 2+ years of experience (preferably in long-term care), are required. The position is full-time, in-person, across statewide PALTC facilities, offering comprehensive benefits.
