APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA who provides independent, comprehensive medical care in PALTC settings.

Goals include improving care quality, patient outcomes, and experiences, while reducing hospitalizations and costs.

Responsibilities encompass conducting exams, diagnosing/treating illnesses, developing care plans, and providing preventive services.

They work autonomously within scope, collaborating with facility leadership and interdisciplinary teams to ensure holistic care.

Key duties involve maintaining compliance, accurate documentation, participating in quality initiatives, and promoting safety and health education.

Qualifications require a relevant degree, at least 2 years of experience (preferably in long-term care), and current APRN or PA licensure with board certification.
