APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant who works independently to manage patient care in long-term care settings.

Key responsibilities include:

  • Conducting exams, diagnosing, treating illnesses, prescribing medications, and developing care plans.
  • Providing preventive care and managing chronic conditions.
  • Working autonomously within scope, making clinical decisions, and collaborating with facility leadership and interdisciplinary teams.
  • Ensuring compliance with regulations, maintaining accurate documentation, and participating in quality improvement initiatives.
  • Participating in infection control, health education, and safety protocols.
  • Utilizing EHR and telemedicine tools effectively.
  • The role requires strong clinical decision-making, communication skills, and the ability to work independently in a fast-paced environment. Qualifications include a relevant degree, 2+ years of experience (preferably in long-term care), board certification, and active APRN or PA licensure.
