APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA who independently manages patient care in PALTC settings, collaborating with facility staff to improve outcomes, enhance the patient and family experience, reduce preventable hospitalizations, and lower costs.

Responsibilities include conducting exams, diagnosing and treating illnesses, developing care plans, providing preventive services, and managing chronic conditions. The AP works autonomously within scope of practice, making clinical decisions and acting as the primary provider, while fostering strong relationships with facility leadership and interdisciplinary teams.

They ensure compliance, maintain detailed documentation, participate in quality initiatives, and oversee health and safety protocols. The role requires excellent communication, decision-making, and technology skills, along with physical ability to respond quickly.

Qualifications include a relevant degree, at least 2 years of experience (preferably in long-term care), board certification, and active APRN or PA licensure. The position is full-time, in-person, with benefits like health insurance, retirement plans, and paid time off.
