APRN/PA – Post-Acute and Long-Term Care – Advanced Practitioner

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA responsible for independently managing patient care in long-term care facilities.

  • Responsibilities include conducting exams, diagnosing, treating illnesses, developing care plans, and providing preventive services.
  • Work collaboratively with facility staff, building strong relationships with leadership and interdisciplinary teams.
  • Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement.
  • Provide health and safety oversight, educate staff and patients, and work independently in urgent situations.
  • Proficient in EHR and telemedicine tools; possess excellent communication and decision-making skills.
  • Qualifications include a relevant degree, at least 2 years of experience (long-term care preferred), and board certification. In-person work at statewide facilities with benefits offered.
