The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA responsible for independently managing patient care in long-term care facilities.
Responsibilities include conducting exams, diagnosing, treating illnesses, developing care plans, and providing preventive services. Work collaboratively with facility staff, building strong relationships with leadership and interdisciplinary teams. Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement. Provide health and safety oversight, educate staff and patients, and work independently in urgent situations. Proficient in EHR and telemedicine tools; possess excellent communication and decision-making skills. Qualifications include a relevant degree, at least 2 years of experience (long-term care preferred), and board certification. In-person work at statewide facilities with benefits offered.
