APRN/PA – Post-Acute and Long-Term Care – Advanced Practitioner

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently to manage patients’ medical needs in long-term care settings.

Responsibilities include conducting exams, diagnosing, treating, prescribing medications, and developing care plans to improve care quality, outcomes, and patient experience while reducing hospitalizations and costs.

They collaborate with facility staff, build relationships with interdisciplinary teams, ensure compliance with regulations, and maintain accurate documentation.

The role requires strong clinical decision-making, excellent communication, proficiency with EHR and telemedicine, and the ability to work independently in a fast-paced environment.

Qualifications include a relevant degree, at least 2 years of experience, board certification, and an Arkansas APRN license. Benefits include health, dental, vision insurance, retirement plans, paid time off, and more.
