Role Overview:
Provides direct patient care in an ambulatory setting, collaborating with providers and the care team to develop and implement personalized care plans that promote high-quality, cost-effective outcomes. Supports clinical staff and assists providers in daily patient management while ensuring compliance with clinical protocols and documentation standards.
Key Responsibilities:
- Assess and evaluate patients’ physical, psychosocial, and emotional needs using critical nursing skills.
- Develop, review, and update care plans in coordination with PCPs and specialists to maximize health outcomes.
- Educate patients and families through verbal and written instructions, assessing their understanding and addressing barriers.
- Support compliance by following clinical protocols and documenting assessments and treatments accurately.
- Work in a safety-sensitive environment, demonstrating alertness and safe practices at all times.
Qualifications & Skills:
- Education: Nursing Diploma/Associate’s degree or higher; RN licensure; BLS certification.
- Experience: 2-5 years preferred; supervisory experience not required.
- Preferred: Bachelor’s degree in nursing and 5-10 years of experience.
