The Ambulatory Clinic RN Care Coordinator is a licensed registered nurse providing ethical, evidence-based, and compassionate care in a breast surgery oncology clinic.
Responsible for managing patient flow, triage, and direct care, ensuring patient safety and high-quality nursing services.
Acts as a liaison among front desk staff, nursing team, physicians, and patients’ families, supporting care coordination and communication.
Develops care plans, sets patient outcomes, and promotes self-management, especially for high-risk or complex patients.
Provides health education, monitors patient conditions, documents in EMR, and educates patients on medications, procedures, and wellness strategies.
Coordinates continuity of care post-hospitalization or ER visits, managing patient needs holistically.
Requires a Bachelor of Nursing, 2+ years RN experience, and active multi-state RN licensure or in-progress application.
Must hold BLS certification, demonstrate strong critical thinking, and possess basic technical skills.
This full-time role supports patient-centered care in a collaborative clinical environment.
