Advanced Practice Professional – Live Well Homecare

We are seeking a responsible and energetic Physician Assistant or Advanced Practice Nurse to join our nephrology practice caring for patients with kidney diseases and dialysis in the Madison area and surrounding communities.

Collaborate with physicians to manage dialysis and kidney disease patient care in outpatient settings.

Provide high-quality, cost-effective care while supporting the care team.

Travel to dialysis locations with mileage reimbursement; schedule is Monday–Friday, 8-hour days, with no weekends or after-hours calls.

Minimum licensure includes Wisconsin NP or PA license, prescriptive authority, DEA license, and relevant certifications. Recent graduates encouraged to apply.

Salary ranges from $120,000 to $130,000 annually with benefits such as health insurance, 401(k) with matching, license reimbursement, paid time off, and professional development support.
