Acute Care Clinical Pharmacist

Role Overview: The Pharmacist delivers safe, effective, and cost-efficient medication therapy, developing and monitoring evidence-based pharmacotherapy care plans focused on patient outcomes and quality of life.


Key Responsibilities:

  • Provide pharmaceutical care within assigned patient care areas, ensuring resource coordination for efficient service.
  • Interpret and clarify physicians’ medication orders, review patient medication profiles for safety and appropriateness, and suggest formulary alternatives.
  • Consult with medical staff on drug therapy issues, monitor laboratory results, and educate healthcare professionals with drug information.
  • Conduct patient discharge medication counseling, participate in medical rounds, and calculate drug dosages based on pharmacokinetics.
  • Verify orders, prepare compounded sterile medications, and report supply needs.
  • Supervise pharmacy staff, including technicians, students, and interns, ensuring accuracy and compliance with standards.
  • Collaborate with healthcare teams to resolve pharmacy-related issues and assist in updating departmental policies.

Work is performed in a clinical setting, requiring adherence to policies and procedures. The role demands a Bachelor’s Degree in Pharmacy (Doctoral preferred) and involves leadership, communication, and clinical skills essential for optimal patient care.
