The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time APRN or PA providing independent, collaborative medical care in long-term care facilities.
Manage patients’ comprehensive health needs through exams, diagnoses, treatments, and preventive services, focusing on improving outcomes and reducing hospitalizations. Work autonomously within scope, making clinical decisions and escalating as needed, while building strong relationships with facility leadership and interdisciplinary teams. Ensure compliance with regulations, maintain accurate documentation, and participate in quality initiatives. Educate staff and patients on health promotion, ensure safety protocols, and utilize EHR and telemedicine tools.
