Role Summary
Provide patient-centered nursing care in a Hospital at Home setting by applying the nursing process to assess patient needs, develop and adjust individualized care plans, administer prescribed treatments, and coordinate effectively with patients, families, and the multidisciplinary care team.
Key Duties
- Perform comprehensive assessments and identify patient care needs.
- Create, implement, and update care plans based on ongoing evaluation.
- Administer treatments safely and monitor patient response.
- Coordinate care communication across patients, families, and healthcare team members.
- Maintain accurate electronic health records and documentation.
- Promote patient safety and deliver evidence-based practice.
- Support quality improvement activities to enhance care outcomes.
