Role Overview
The Wellness Nurse LPN is responsible for monitoring resident health, supporting medication management, and ensuring clinical quality within a senior living community. Key duties include conducting assessments at move-in, routine wellness visits, and during change-of-condition events; monitoring health trends; following up on incidents such as falls and hospital returns; transcribing physician orders; completing medication audits; and maintaining compliance with infection control and regulatory standards. The role involves collaborating with families, physicians, and care staff to promote resident wellbeing through proactive assessment and early intervention.
Qualifications & Skills:
- Active LPN license in good standing
- Graduate of an approved nursing program
- Minimum of 1 year experience in senior care or clinical settings
- Strong assessment, documentation, and communication skills
