Role Overview

The Wellness Nurse (LPN/LVN) supports resident health, monitors care needs, and ensures medication safety and clinical compliance in a senior living environment.

Key Responsibilities:

• Conduct wellness assessments and monitor health changes
• Support medication management and perform MAR/TAR audits
• Communicate with families, physicians, and care teams
• Monitor incidents, falls, and follow-up care
• Maintain documentation and ensure regulatory compliance
• Support infection control and clinical quality initiatives
• Train and support care team members

Qualifications & Skills:

• Active LPN/LVN license
• Over one year of experience in senior living or clinical settings
• Strong assessment, documentation, and communication skills
• Knowledge of medication administration and compliance
• Excellent problem-solving abilities
