The LPN Unit Care Coordinator supervises and manages patient care, ensuring adherence to laws, regulations, and standards.
Must have completed an accredited LPN program and hold an active, in-good-standing LPN/LVN license.
Preferably has at least one year of geriatric nursing experience.
CPR certification required upon hire or during orientation, and must remain current.
Possesses advanced knowledge of practice, makes independent decisions, and understands relevant laws and procedures.
Implements departmental policies, maintains confidentiality, and adheres to ethical standards.
Effectively leads daily nursing activities, manages patient care plans, and coordinates services with RNs.
Administers medications accurately, responds to emergencies, and ensures high-quality patient care.
Demonstrates professionalism, excellent communication, teamwork, and dependable attendance.
Performs physical activities such as lifting and movement during shifts.
Fluent in English, with strong reasoning, judgment, and interpersonal skills.
