Unit Care Coordinator (LPN)

The LPN Unit Care Coordinator supervises and manages patient care, ensuring adherence to laws, regulations, and standards.

  • Must have completed an accredited LPN program and hold an active, in-good-standing LPN/LVN license.
  • Preferably has at least one year of geriatric nursing experience.
  • CPR certification required upon hire or during orientation, and must remain current.
  • Possesses advanced knowledge of practice, makes independent decisions, and understands relevant laws and procedures.
  • Implements departmental policies, maintains confidentiality, and adheres to ethical standards.
  • Effectively leads daily nursing activities, manages patient care plans, and coordinates services with RNs.
  • Administers medications accurately, responds to emergencies, and ensures high-quality patient care.
  • Demonstrates professionalism, excellent communication, teamwork, and dependable attendance.
  • Performs physical activities such as lifting and movement during shifts.
  • Fluent in English, with strong reasoning, judgment, and interpersonal skills.
