Job Summary
Provides patient evaluation and assessment to establish functional, measurable goals aimed at restoring abilities. Delivers treatment based on a written plan of care, monitors progress, and adjusts interventions as needed. Responsible for discharge planning, patient and family education, and documenting care in medical records. May supervise clinical students, support staff, and volunteers, fostering a caring environment that promotes health and healing. Performs additional duties as assigned.
