Job Summary
This role involves evaluating and assessing patients to establish functional, measurable goals aimed at restoring abilities. The provider develops and implements a treatment plan, monitors progress, and adjusts care as needed. Key Responsibilities
- Assess patients based on subjective and objective data.
- Create and follow a written care plan.
- Deliver treatment aligned with care goals.
- Evaluate treatment effectiveness and reassess patient status.
- Provide discharge planning, education, and community referrals.
- Support staff and student development.
- Participate in quality improvement activities.
- Complete assigned tasks efficiently.
Qualifications & Skills
- Entry-level experience preferred.
- Basic skills necessary.
- BLS certification required within 14 days of hire.
