Role Overview:
The National On Call advanced practice clinician (APC) provides telehealth care, guiding patients, caregivers, and facility staff 24/7, including holidays. This remote, part-time role involves delivering virtual acute, chronic, and behavioral health assessments, documenting findings per regulatory standards, and utilizing EMR systems efficiently.
Key Responsibilities:
Coordinate care through telephonic and video platforms, ensuring patient privacy.
Manage acute and chronic conditions collaboratively with physicians and specialists.
Conduct comprehensive assessments, document accurately, and ensure quality standards.
Facilitate care transitions, monitor patient needs, and support care plan adjustments.
Maintain excellent customer service and communication with all stakeholders.
Qualifications & Skills:
Master’s or Doctorate in Nursing with active, unrestricted NP license and national certification (FNP, ANP, GNP, AGACNP).
DEA licensure or ability to obtain post-hire.
Proficient in EMR documentation and virtual communication tools.
Minimum of 3 years clinical experience as an APC preferred.
Ability to work flexible shifts, including weekends and holidays.
