Telephonic Nurse Practitioner (Part-Time) – OH, MO, WI or IN License Required

Role Overview:
The National On Call advanced practice clinician (APC) provides telehealth care, guiding patients, caregivers, and facility staff 24/7, including holidays. This remote, part-time role involves delivering virtual acute, chronic, and behavioral health assessments, documenting findings per regulatory standards, and utilizing EMR systems efficiently.

Key Responsibilities:

  • Coordinate care through telephonic and video platforms, ensuring patient privacy.
  • Manage acute and chronic conditions collaboratively with physicians and specialists.
  • Conduct comprehensive assessments, document accurately, and ensure quality standards.
  • Facilitate care transitions, monitor patient needs, and support care plan adjustments.
  • Maintain excellent customer service and communication with all stakeholders.

  • Qualifications & Skills:
  • Master’s or Doctorate in Nursing with active, unrestricted NP license and national certification (FNP, ANP, GNP, AGACNP).
  • DEA licensure or ability to obtain post-hire.
  • Proficient in EMR documentation and virtual communication tools.
  • Minimum of 3 years clinical experience as an APC preferred.
  • Ability to work flexible shifts, including weekends and holidays.
