We are seeking a PRN Home Health Speech Therapist / Speech Language Pathologist to join our dedicated team.
Key responsibilities include:
- Assessing and evaluating patient needs related to speech, language, swallowing, and cognitive impairments
- Developing and implementing individualized treatment plans in coordination with the healthcare team
- Providing interventions to improve communication and functional abilities
- Monitoring, documenting, and reporting patient progress and responses to treatment
- Educating patients, families, and caregivers on therapy techniques and strategies
- Delivering services in various home settings, ensuring respectful and effective care regardless of patient background
Qualifications include:
- State licensure in Speech Therapy
- Valid driver’s license, vehicle insurance, or access to reliable transportation
Preferred qualifications:
- CPR certification or ability to obtain within 90 days
- Availability to work a weekend rotation
